New Patient Referrals 

In order to assist our referring physicians and their referral coordinators, Dr Alexis and Dr Kidd have created the following checklists.

Please include the necessary information in your referral so that we can care for your patients efficiently and effectively.

COLONOSCOPY REFERRALS

  • Recent progress note(s) with active/important medical issues addressed.

  • Comprehensive problem list/past medical history

  • Current insurance information and demographics

  • Authorization, as needed by patient's health plan

  • If patient has had a colonoscopy in the past, we need as much information as possible:

    • Colonoscopy report(s)

    • Pathology report(s), if applicable

    • If no reports available, date of last colonoscopy, including month and year ***This information is critically important! Most health plans will deny coverage if date criteria are not met.***

 

  • All referrals for diagnostic colonoscopy (rectal bleeding, +FIT/+Cologuard, iron deficiency anemia, changes in bowel habits, etc) are seen in the office or via telehealth prior to scheduling.

 

  • Some screening colonoscopy referrals are seen in the office or via telehealth; some are “DIRECT SET UP”, i.e. no pre-procedure consultation is required.

    • To have your patient considered for a “DIRECT SET UP” SCREENING COLONOSCOPY:

      • Dr Alexis and/or Dr Kidd will review the referral, schedule the patient for their procedure, and will meet the patient in pre-op holding at SVH.

      • These types of “direct set up” procedures are usually reserved for healthy patients being in need of second or subsequent colonoscopy for:

        • average-risk screening, z12.11

        • screening due to a family history of colon cancer, z80.0

        • screening due to personal history of colon polyps, z86.010

 

  • Patients who require an EGD, or if an EGD is requested to be added on to the colonoscopy, should be directed to Dr Alexis. Many, but not all, patients will be able to have their EGD and Colonoscopy performed in the same procedure.

BREAST REFERRALS

  • Recent progress note(s) with active/important medical issues addressed.

  • Comprehensive problem list/past medical history

  • Current insurance information and demographics

  • Authorization, as needed by patient's health plan

  • Breast Imaging for the past 12 months. If imaging was performed at SVH or QVMC/PVH/Memorial (Providence facilities), Dr Alexis is usually able to view the images directly.

  • Pathology report, if available.

GENERAL SURGERY & COLORECTAL REFERRALS

  • Recent progress note(s) with active/important medical issues addressed.

  • Comprehensive problem list/past medical history

  • Current insurance information and demographics

  • Authorization, as needed by patient's health plan

  • Imaging: relevant imaging for the past 12 months. If imaging was performed at SVH or QVMC/PVH/Memorial (Providence facilities), Dr Alexis and Dr Kidd are usually able to view the images directly.

  • Pathology report, if applicable

REFERRAL WORKFLOW

  • Once the checklist is complete, please fax to our office: 844-581-1700

  • Our patient services coordinator will process the referral so the surgeon may review it.  Please allow 1-3 business days.

    • Direct Set Ups will be contacted to complete their online intake forms and to schedule their procedure​

    • Other patients will be contacted to schedule a consultation appointment either at our office on Perkins Street or via doxy.me (telehealth)​​

    • Your patient is welcome to call after 3 business days to schedule their appointment.

    • You may also invite them to visit their surgeon's website to find links to our online intake forms. These are required prior to the patient being seen by the surgeon and prior to scheduling a "direct set-up" colonoscopy.

  • Communication back to your doctor:​

    • After the patient is seen for a surgical consultation, the surgeon will send her H&P or progress note back to the referring provider via fax.

    • After the patient's colonoscopy, the surgeon will send the operative note back to the referring provider via SVH Epic. ​
