Additional Fees:
There is an additional $25 fee for completion of forms for use outside of the patient's medical record, including Family Medical Leave Act and EDD.
Medical records can be sent to providers outside of your care team for a nominal fee, based on number of documents requested.
No-Show Fees: In order to prevent disruptive cancellations, we request ample notice if you need to reschedule an office visit or an operation/procedure. Details of this policy are available on our patient intake forms.
